A big prize could be coming to the city of Stamford, a city taking the lead in promoting health and eliminating disparities.

"The Robert Wood Johnson Culture of Health Prize recognizes the fact that we've worked with our partner, Stamford Hospital, and also a number of other service providers to create a model for Health," said Vin Tufo, the CEO of Charter Oak Communities.

Tufo has been a key player in Stamford's Vita, the program at the center of this nomination.

"Vita means health in Latin.  And the Vita Health and Wellness District is really looking at the West Side Community as place we can make healthier," he described.  "A good example of that is a farm we've created on Stillwater Aveue called Fairgate Farm.  This is a communal, urban agricultural learning platform that brings many people together who have no experience gardening."

And, with each step forward, the program continues to gain national attention.  Already it's recieved funding from the Environmental Protection Agency and the Housing and Urban Development.

"We don't just do this as a job, we do this to make a difference," said Tufo.

Fifty communities, including Stamford, are competing for this prize.  There will be six winners for a prize of $25,000 each.  Stay with It's Relevant for the results.
